A late-night medical helicopter mission turned tragic in Cookeville, Tennessee, when the aircraft crashed during an emergency flight.
The Air Evac Lifeteam helicopter went down just before midnight on Friday. Local rescue teams rushed to the scene after nearby residents reported seeing flames and hearing a loud explosion.
Because of the difficult terrain and poor visibility, it took firefighters nearly 30 minutes to reach the wreckage.
🚁 What Happened
The helicopter was on its way to pick up a patient when it suddenly lost contact with air traffic controllers.

According to Putnam County Emergency Services, the aircraft crashed in a wooded area about five miles from Highway 70. Witnesses said the impact created a fireball visible from nearby homes.
Investigators from the Federal Aviation Administration (FAA) and the National Transportation Safety Board (NTSB) arrived early Saturday morning. They’re reviewing flight data, weather conditions, and pilot communication records.
Meanwhile, recovery teams are working to remove the wreckage safely and preserve evidence.
🩺 Crew Casualties and Survivors
Officials confirmed that one medical crew member died at the scene, while the pilot and flight nurse survived with serious injuries.
Both survivors were flown to Vanderbilt University Medical Center for treatment. Hospital staff reported that they remain in stable condition.
Air Evac Lifeteam released a statement expressing sorrow and gratitude for the first responders who reached the scene.
“Our hearts are broken. We lost a brave teammate who dedicated their life to helping others,” the company said.
🌧 Weather and Flight Conditions
Investigators noted fog and low visibility over the Cookeville area that night.
Because of those conditions, the helicopter’s pilot may have struggled with reduced sightlines. However, officials haven’t ruled out a possible mechanical malfunction.
In addition, FAA officials confirmed that the helicopter’s maintenance checks were up to date earlier this month.
⚙️ Ongoing Investigation
The NTSB expects to release a preliminary report within two weeks.
Because this type of crash involves both mechanical and human factors, full results could take several months. The FAA will analyze black box data and communication logs to understand what went wrong.
Meanwhile, local authorities have blocked access to the crash site to protect evidence while cleanup and inspection continue.
💬 Community Response
Residents across Cookeville are mourning the loss. Community members have gathered near the local Air Evac helipad, leaving flowers and candles in memory of the fallen crew member.
Tennessee Governor Bill Lee also shared condolences, posting:
“We grieve with the family of a selfless first responder who gave their life in service to others.”
Because of the emotional toll, counselors have been made available for emergency staff and flight teams affected by the tragedy.
🕯 Honoring Their Service
Every day, air medical crews risk their lives to save others. This crash serves as a heartbreaking reminder of their courage and sacrifice.
The Tennessee community continues to honor the victim and support those recovering. As investigations move forward, many hope safety improvements can prevent another tragedy like this one.
